Here’s a link to the Stress First Aid instructors manual (2018) from the U.S. Forest Service Department of Agriculture, directed at Wildland Firefighters. It explains the original diagnostic tool cited under the adapted graphic above (Watson et al. (2013)), which was based off a model developed by the Navy / USMC for identifying and mitigating combat stress / potential PTSD.
